Home Health
Meadowood's Home Health Agency is licensed by the Commonwealth of Pennsylvania's Department of Health, Division of Primary Care and Home Health Services. Our services are reimbursed by Medicare. The agency serves residents of Meadowood's Continuing Care Retirement Community who require comprehensive, intermittent, hospital-type professional and supportive services which are not custodial and do not demand 24 hours of continuous care.
In the privacy of your home, a Registered Nurse will provide nursing care treatments as prescribed by your physician. The nurse will also coordinate your care between you, your physician and other members of the health care team. A Certified Home Health Aide will assist you with personal care and Activities of Daily Living (ADL's). Our dedicated team of therapists offers clinical excellence in all areas of Physical, Occupational, and Speech Therapies. An in-house Social Worker can assist you in accessing community programs and other resources.
With your permission, a designated family member(s) will be included in our Family Connections Program. Our goal is to provide open communication between you, your family and caregivers, to discuss resident goals, concerns, suggestion and successes. Your will always be the first decision maker for all care needs, unless you choose to defer to your family.
The Home Health Agency office is located on the lower level of the Schulz Community Center. Please contact Lynn Plepis, RN, at (610)584-3633 for any information regarding Home Health Services.

With an extensive health center renovation to be completed in 2009, most rooms will be private with in-room showers. Individualized care plans are developed to meet each resident's needs by a professional team which includes licensed nurses, a registered dietician, a social worker, a director of recreational programming, and other rehabilitation staff as appropriate. Meals are served in a lovely dining room and residents have access to many daily recreational activity options offered within Holly House and throughout the Meadowood campus.
Meadowood residents may desire a short-term stay in Holly House after a hospital stay, surgery, or a debilitating illness. The goal of the Health Center rehabilitation team is to work with each short term resident to restore them to their highest functional level and support their safe return to home. Physical Therapy, Occupational Therapy, and Speech Therapy are available as ordered by the resident's physician. The Health Center social worker will coordinate a short term resident's safe discharge to home as appropriate with follow up services through Meadowood Home Health and Resident Services.
