Located on the Meadowood campus in Worcester, Pennsylvania, Holly House provides compassionate Skilled Nursing services for individuals who are temporarily or permanently in need of 24-hour health care services. Our licensed Skilled Nursing staff works with residents and their families, as well as the rest of our team, to develop individualized care plans that meet the needs of each person they serve. Our team of experts includes:

Our top priority is to provide exceptional Skilled Nursing care that promotes dignity and respect in a warm, comfortable atmosphere. Most accommodations are private and offer in-room showers to provide optimal comfort for our residents. Meals are served in a lovely dining room and residents have access to many daily recreational activities offered within Holly House and throughout our continuing care community.

Meadowood is proud that telemedicine is a part of its comprehensive, holistic approach to care. Curavi Health, the company that provides telemedicine to Meadowood, has an interesting history.
Curavi Health was born out of the realization that patients in our health care system face many challenges during treatment, and one of the biggest is getting access to high-quality care — quickly and comfortably. The founders saw the lack of physician care after-hours frequently leading to negative patient outcomes. The addition of this service will enable our residents and short-term Rehabilitation patients to have a check up from a physician 24 hours a day, rather than take an unnecessary trip to the emergency room. Communities where Curavi Health is in place have found that their residents can avoid a hospital trip 80 percent of the time!
So, what does this mean? Imagine you are a resident and recuperating in the Skilled Nursing area of Meadowood. It is 2 a.m., and you are not feeling well. Instead of making a trip to the emergency room, the charge nurse is able to have one of the Curavi Health physicians “see” you via computer monitor and assess you on the spot! This is how it works:
- A nurse, recognizing that a patient is in need, calls Curavi Helath.
- The nurse reaches a doctor and they review the patient’s clinical issue(s).
- Pushing a single button activates the telemedicine unit and brings a doctor to the patient’s bedside.
- The patient and physician meet and an examination takes place.
- The physician and nurse begin care, with the inclusion of the patient’s attending physician + family as needed.
- TripleCare provides full documentation of the visit to ensure coordination of care.
- The patient is comfortably treated in place and a hospitalization is avoided.

Home Health Services
At Meadowood, we embrace the philosophy of aging in place. Our Home Health agency serves residents of Meadowood’s Life Plan Community who require comprehensive health care services, but not at the level provided in Skilled Nursing. Our agency is licensed by the Commonwealth of Pennsylvania’s Department of Health, Division of Primary Care and Home Health services, and our in-home nursing care is reimbursed by Medicare.
In the privacy of your home, a registered nurse will provide nursing care as prescribed by your physician. Your in-home nurse will also coordinate care among you, your physician and other members of the Health Center team. In addition to nursing services, skilled Rehabilitation services are also available (physical therapy, occupational therapy and speech therapy).
This team approach allows for open communication among you and your caregivers to discuss goals, concerns and suggestions. In all cases, you will remain the primary decision maker for your health care needs, unless you wish to defer to your family.
