After being discharged for a hospital stay, surgery or illness, Meadowood residents and people from the greater community have access to a short-term stay for skilled nursing rehabilitation in Montgomery County.

Specialized Support for Stroke and Cardiac Patients

A variety of rehabilitation and post-acute care and senior rehab services, including injury, orthopedic, stroke, and infection and heart recovery, are available to carry out the orders of residents’ physicians, including physical, occupational and speech therapy services for seniors.

Based on recent data, Meadowood’s short-term senior rehabilitation services is a high-performance rehab center, earning a high-performing 3 out of 3 rating in U.S. News & World Report’s ranking. The rating evaluates a nursing home’s quality of post-acute care services for patients recovering from a hospital stay such as after stroke, heart attack, infection or accidental injury.

Safety First: Exceptional Post-Acute Recovery Care

Working together, our accredited therapists and nursing staff use an integrated team approach to develop personalized care plans to improve overall wellness and promote the highest level of functional independence. When residents complete their Senior Rehabilitation, our Health Center social worker will help coordinate a safe discharge home, as well as follow-up services through Meadowood Home Health services.
